Thank you for allowing us to care for you. Due to circumstances outside of our control, our clinic is no longer providing medical services. Please see below for more information:

Medical Records:
To request your medical records, please email info@hometownurgentclinic.com. All medical record requests must be submitted in writing in accordance with HIPAA (45 C.F.R. § 164.524). Please allow up to 72 hours for a response. All records will be maintained in accordance with Mississippi and federal medical record retention laws.

​

Medication Refills:
Established patients may request a one-time, short-term refill when clinically appropriate and permitted by law. This is intended only to allow time to establish care with a new healthcare provider. Ongoing treatment and future prescription needs must be managed by a new primary care provider.

​

Emergencies:
If you are experiencing a medical emergency, please call 911 immediately.

To the Union community and the surrounding areas,

It is with a deeply heavy heart that I share that Hometown Urgent Care & Family Medicine has permanently closed as of November 14, 2025. This has been one of the hardest moments of my career, and I want to speak to you honestly and from the heart. This closure was sudden, unexpected, and brought on by circumstances outside of my control. It was not the path I ever planned, nor the decision I would have made for our patients, our staff, or this community. I had envisioned a different transition for our clinic, but circumstances shifted and the decision was taken out of my hands.

Since the day we opened, my heart has been in this clinic. I built this practice with the hope of serving Union with compassion, dignity, and the highest level of care I could give. I poured myself into every detail—every policy, every patient, every long night—because caring for this community has been one of the greatest honors of my life. I also want to reassure you that this decision does not reflect the care, integrity, or reputation of our clinic or our staff. We have always taken great pride in providing safe, compassionate, and ethical care to every person who walked through our doors. Your trust was never misplaced.

While our doors have closed, I remain committed to supporting our patients through this transition. Medical records are available upon request, and short-term medication refills may be provided when clinically appropriate to help you establish care with a new provider. You will not be left without a path forward. This community embraced me, believed in me, and trusted me during some of the most meaningful years of my career. I am heartbroken that this chapter is ending earlier than I ever wanted, but I am profoundly grateful for every patient who allowed me to be part of their story.

Thank you for your kindness, your support, your prayers, and for letting Hometown Urgent Care be a part of your lives. I will carry this experience with me always.
